Quick release started to fail after 3 or 4 years

I was quite happy with this vise for the first 3 or 4 years. Then the eccentric nut began to slip when attempting to loosen the vise. Not sure if it's actually a nut, maybe the vise has some play that allows the nut to click making it difficult to loosen the grip on the vise. I've tried to troubleshoot the issue but nothing I've tried seems to help. Now that the vise is about 5 years old, they are practically unusable. I give 2 stars because it worked great for a while. I've been using them for the occasional woodwork job with hand tools for almost four years. This is a classic design that works very well. My vise is maple lined on both jaws, plus some leather on the outer jaw. There is one major downside I see with the vise and that is the quick release mechanism. Both the product description and the manual state that this vise is designed for a 2-1/4" Workbench without upholstery fits. That is not right. I did a test fit and found it wouldn't sit flush with my 2-1/4" countertop so I tried a 1/4" spacer and it looked fine. I installed it with this washer and it STILL proud of about 1/32" of the hole.
