It broke the first time you put it in reverse.
It broke in half on the first shift. It's cheaply made and poorly designed. I really like the skull itself so I sanded the hole a bit and used JB welding to put some nuts in the skull. After drying, I placed the skull directly on the switch. So the adapter part is no longer on it. And it will never break or slip.
