Toto Supreme series toilet flap is outdated and leaking. After looking at the part number I found that part numbers THU140S and THU275S matched this style and both have been changed to THU499S. This flapper was the cheapest I could find, it was a newer version replacing older versions. It is identical in shape and size to the original. Replacing the shock was easy as the two grips are soft and slide onto the two side pins. The chain was a little long, but it's easy to adjust how long you want the flap open and water to pass through.
