Mismatched Hex Holes
I needed 3mm set screws that could be tightened fairly tight. Using a quality US made 1.5mm hex wrench (Bondhus) showed that the hex holes in about half of the 3mm screws were oversized. Some were downright sloppy. The result is key slip and of course no tightness. I don't know other sizes.
