Usually you need to adjust these settings right out of the box, no big deal (delivery, temperature, humidity, assembly line norms etc.). This micrometer was immediately proven accurate when I tested it on a new engine piston after testing it with the 4" gauge block that came with the kit. I bought this micrometer because the accuracy is 0.0001 which is the most important. my nameless micrometers are accurate to 0.001. I was pleasantly surprised at the accuracy of this outside micrometer. I didn't expect it to live up to the claimed 0.0001 inch accuracy. After adjusting the 0-point scale and vernier scale (they were just a hair out of the box), I tested Anytime with multiple units of measurement, and they all turned out to be excellent. The micrometer runs very quietly and is extremely precise. Dimensions are in inches with a decimal point, although the instructions state dimensions in millimeters (mm). If you want to express the value in millimeters or fractions of an inch (e.g. 1/8 inch), you need to calculate it. I would give 5 stars if the instructions for use were clearer.
