Material failure almost caused serious injury

We bought this swing more than 4 months ago! We would like! We use it daily and have never hit the weight limit. We were having a party next door and it suddenly collapsed with a little girl on top! With an estimated carrying capacity of 500 pounds, this girl only weighed about 70 pounds. My husband ran over to find the nut had come loose from the bolt and was out of order. These are the materials that came with the swing. I had to update the attachments.
