I have a very wide neck (18-19) and this collar was too small in diameter to be practical. It kept my neck perfectly straight during my 9 hour flight while I was awake, but choked me when I fell asleep. There is also an internal plastic ring which I believe is meant to stiffen the collar and help it hold its shape. After a 9 hour flight the inner ring seems to have shifted and moved a little in the collar. I WILL SAY this is probably one of the best sleep collars out there if you have a "regular" or "small" neck. It's very stiff and has a small dimple where the chin rests, which adds to the comfort. It is soft but stiff and a long velcro strap is used on the back of the collar to keep it around the neck.
