The pieces of fabric that the Velcro is attached to are either too small or poorly positioned.
The fabric for velcro, which can be folded to close, is too small, so when washed they catch and sometimes ruin other things. perfect with all the wiggle room you need but the wrapper part is too big. The fabric that the Velcro straps are attached to is not wide enough to wrap my baby's arms tightly. He often slips in it when moved, or moves on his own as a result, which seems dangerous. The fact that it fits him correctly with everything else makes me suspect the piece was sewn in the wrong place.
