My waist is exactly 48". I have a tape measure so I'm sure. I ordered a size 50 on top. up to the 3rd [middle] hole. "I agree when I measured the belt, I got exactly 50" this way. But why would you need these measurements? When you wear a belt, the hole that the prong goes through is on the OUTSIDE end of the buckle. If I take this belt from the outer end of the buckle, the distance to the 5th (last) hole is 49". This belt is too big for me or anyone else with a 48" waist. I'm looking for a new belt that needs new holes to make it smaller and this will give me something like 7" overage where I have no idea about the other reviews where people said they need 2 or 3 sizes up, other than that they were from a few years ago. Reading helped me get the size wrong.
