Attention, medium doesn't fit 40 inch, if you want 40 inch you have to order big. You can get away with this size for 38 inches. My husband has a 35" kilt and this strap did not flex as much as suggested with the velcro. Also, the crease for attaching the buckle wasn't formed properly, I actually stepped on it to get a flat crease and it still didn't fold well. The quality isn't very good (very thin and hard) and I don't expect it to last long, but it made it through the event we need it for. Hope that helps. :-)
