with center indentation for head/neck support and small horizontal channel and center opening, this seemed like a near perfect design to me - I have neck stiffness/pain and migraines and earaches from wearing earplugs for 30 years. I tried the women's size, then the toddler size because I really wanted to love this pillow, but the firmness while sleeping on my side was too uncomfortable for my ears. if the middle sides were more padded and softer and only the middle was hard, that would be ideal. Also, despite the lady's reduced size, I did not have enough size and the baby is too small / low. Special note: I found the pillow a bit softer without the included cover - it definitely adds stiffness and firms the pillow. (I had to use a small flathead screwdriver to open the center latch - couldn't manually - close easily.) Not for me, but great design for hard pillow lovers and those who can tolerate the pillow height.
