Completely waterproof. The bottom feels like plastic wrap and the top feels like synthetic fabric. You'd think the fabric part would get at least a little wet, but no, as you can see in this video I made in my kitchen sink, the water just clumps and rolls off like water off a duck's back. Of course, it's not breathable, so it's best to cover it with a comfortable breathable mattress topper and then a sheet. You don't have to worry about the mattress topper being waterproof as this mattress topper will get the job done.
