Caused by high pitched sound from TV
After hours of troubleshooting, changing HDMI cables, inputs, TV settings, input settings and more, I found that this high pitched sound was coming from the TV when playing bright HDR content became. and in particular the playback of HDR content via a CEC-capable HDMI line. I suspect that the increased power required for CEC and especially HDR highlights is beyond the capabilities of this power cord. The high pitched tone I found is due to either voltage or power supply issues. When I went back to the original cable all my problems were gone.
