The top of the dispenser is not designed very well. You really should use it, otherwise you won't be able to control the amount that comes out of the bottle. I finally managed to separate it after screwing it tight to the board and twisting the top. I assume the product has been well tested for safety, although the FDA doesn't test or approve dietary supplements so you're at the mercy of the manufacturer. Probiotics are sometimes recommended for children who have taken large doses of antibiotics. Healthline recommends seeing a pediatrician. I love that it's free of the eight major allergens.
