OK for a while then dies out completely

These chargers work great and are super fast. However, the USB port stopped working after a couple of weeks, but it was fine as it came with two. So I threw this one away and used another new one to find out the cable wasn't working and charge mine. Phone. So I ended up buying another set for my husband and I and ended up with the same thing. Either the port or the cable stopped working after a few days/weeks.
