I took the advice of another reviewer and instead of ordering the next size up (as recommended by the seller), I ordered the same size as my 38" waist. But even that was too big? I had to buy a 36 inch belt even though my waist was 37-1/2 inches. However, I had a loose measuring tape around my waist. The 38" I ordered first was exactly the same length as my old one, but I had to drill another hole in my old one because it was too big. The 36" now fits perfectly, right in the middle of the five holes in the belt. The belt looks nice and natural and doesn't stretch, creating unnecessary tension at the waist.
