My husband always wears a Medium in underwear – he wears a 32 waist in pants (6’, 150 LBS, slim/fit). These "fit” but they are very tiny & very low. There is not enough room for the important bits. He said no way – these would be very uncomfortable. He usually wears bikini style with the pouch design (not the flap door) but these are just too tiny. They are well made & the fabric is soft. The pouch is double fabric.
