In the pictures it looks like the pillow case is the actual pillow but the pillow and case are separate. It's cute and fluffy, very supportive. I don't know how heavy other people's kids heads are but it stays rather structured, supportive and fluffy under my daughter's head. And, she loves the mermaid print.
