We bought this for our cat, she is grown but small and rather thin. We went with the "medium" option because we thought "small" would be kitten size, but the medium is quite large for her, even at the tightest buckle. It stays on pretty well despite the size, it doesn't slip off unless we do it on purpose. Her paw sometimes gets stuck in it, but that's because she's not very smart. If you have a fat cat I would recommend Medium, but if you have a small/skinny cat I would recommend Small. Otherwise it's great and doesn't bother her like other cheap pet store collars.
