How can you shrink wrap a product but not secure the lid?

I was very upset when I opened this package, half of the product went into the mail bag. The bottle was sealed but the cap did not fit tightly on the bottle and more than half of the product spilled, soaking all the other products in the box. I would return it but it's so dirty and good I just threw it away as it ruined everything in the box. Needless to say I'm not very happy. How hard is it to make sure the lid is secure before shrink-wrapping it?
