will not stick to health and safety products

The tape looks great but when we applied it to a clean cement floor it didn't stick. It pulled straight up. The tape didn't have any special instructions so it should clean and dry the surface and stick it to that area. Maybe we have a bad throw, I'm not sure. I don't think it's user error with the tape. We tried sticking it to different things, wood, cement, rubber, metal and it didn't stick to any of them. Disappointing.
