If you have a narrow neck it will rest directly on your collarbone making it very uncomfortable and low. The air only blows on the cheeks, not on the neck. The small holes are closer to the back of the device, you don't feel any air at all. Bought for hot flashes and it's ok but definitely need a smaller size.
