For the purposes of this review I will refer to 'medical' masks as Covid masks. on my face The top strap worked great on its own. I used it to wear a COVID mask outside for a trip to Disney & Universal. The gears next to the top strap allowed me to attach the COVID mask's ear straps to it, making it feel like a single mask. I wear glasses and a hearing aid so the top strap on my head keeps it from getting tangled in my glasses or hearing aid. win-win situation. It fits well with just one strap even when I don't have a COVID mask on.
