I'm a 33 waist and ordered the medium and it fits just fine. Not overly snug, not overly loose.Honestly I didn't know what to expect with this. I have a vasectomy coming up and the doctor told me to order one of these for post surgery support. This fits and seems to offer decent support to the boys, so I think it'll work just fine. It's not scratchy, doesn't pinch anywhere, and the fit is as described. For my uses, that's 5 stars.
