An inexpensive trackball might work for you.

Kensington Orbit Trackball Mouse Optical mouse can be suitable for people with hand problems. The onset of arthritis and tendinitis greatly reduced the amount of time I could spend in front of a computer. Although I'm a longtime Microsoft trackball user, it has helped relieve carpal tunnel syndrome. I eventually found that even the trackball was not helping my arthritis/tendonitis. The Kensington Optical Trackball Orbital Mouse allows the user to "click" without having to bend their thumb or other fingers. Being able to press the button without flexing your finger seems to minimize stress on your joints. I was hoping that using the "Kensington Optical Orbit Trackball Mouse" would relieve my pain and allow me to use the computer without problems. The Kensington Orbit Optical Trackball Mouse felt stiff on the first few uses. It also felt a bit small and the lack of a rolling ring made this product difficult for me to use. Revain's price is about 25% lower than other sites. If you want a budget trackball and can do with a smaller ball and no scroll ring, this might be for you.
