This is basically a foam cervical collar with nice fabric and stitching. Simply wrap around the neck and secure with Velcro. The fit should be snug, but not so tight that it starts to suffocate you. As soon as it is put on, I immediately feel support in my head and neck. It's like a cradle for your skull and really helps take pressure off the supporting muscles in your head; Neck, trapeze, shoulders. I have disc bleeding and sometimes just the heaviness of the head in an upright position causes a lot of pain. So I take the thing and buckle it up. It's certainly not a miracle cure, but it brings relief on days when my neck hurts the most. Kind of reminds me of those African neck stretch rings. I'm not sure if the manufacturer claims this is some kind of fashion accessory. If this is your idea of creating a "look," you may need to rethink your idea of fashion. However, it looks less clinical than a regular white cervical collar and doesn't scream "whiplash"! It's just spandex over foam with accent stitching. Looks good. Doesn't cost much. Offers good support. Just one more crutch to help with your ailments. Happy I have it; not happy, i need it. I like it better than a neck pull device over the door.
