I'm not sure when men's underwear became a fashion item. I now seem to be in a minority: I don't traipse around in my undies. My fans will just have to do without. For me, underwear is about comfort. Normally I wear a medium. But the last few sets of undies I ordered have been tighter than I wanted. So, I ordered a large. I'm glad I did. They fit like the mediums in my normal Fruit of the Loom pairs. Ok, they're still snazzy. So, if I change my mind about running around in undies, I could still do it and make a fashion statement. But I won't be sacrificing comfort. These are mostly cotton. They breathe. Some spandex makes them stretchy. I don't this, but it does not detract. It does mean that they won't shrink. It may also make them last longer.
