From the photos and description, it was difficult to tell how large the ice pack opening was on the back of the pillow. It runs the full length and most of the width of the pillow, so you can use an extra large ice pack (see photo) and relocate it if needed. I was concerned that using multiple small ice packs would cause them to stick together, and I think that might still be the case, so we're planning on using one large gel ice pack as shown. Bought for my daughter's upcoming hysterectomy and tested in the car. The fit is great and the difference in comfort is noticeable. Give a large capacity for a wide and flat ice pack, this should make it easier for everyone to get home from the hospital and for post-operative visits. I wish I had had this many years ago after my abdominal surgery. This pattern is also slightly darker than the picture, which is a pleasant surprise. It's actually quite beautiful.
