Not very secure in TV ceiling and wall mounts

I bought this as a cable box platform and put the platform on top of a flat screen TV. It didn't seem very secure, but I thought it was fine. However, one morning I heard a loud crack and then realized that the shelf and cable box had fallen onto the cabinet that the TV was in. In my setup, mounting this element on top of the TV seemed awkward, which may have contributed to its not being very durable.
