Fell apart after second wash and line dried

Bought this duvet cover for my 3 year old daughter. I washed it on a gentle cycle and hung it on a line to dry before laying it on the bed. Repeat this after about 3 weeks and the duvet and pillowcases will unravel at the seams and have a large hole. Super disappointed and I have a very sad little girl
