What the heck happened to this product?

I've been using this product for two years now, it has always worked so well and the consistency was like white cream when it came out of the tube. This last time came out translucent and grayish, very runny and didn't mix well with the developer. As a result, my hair turned gray! As I said, I've been using this product (I usually mix ice and steel blue) for years and have never had a problem like this.
