Possibly a bad batch of tape
I bought this tape, it lasted less than a month on the machine and it fell off. I cleared and prepared the entire area. I've seen concerns about some from other reviews of this product (not all from this seller) lately. Maybe the problem is a bad tape cartridge, but I don't have any money right now anyway.
