I bought medium for my large (12lb) cat. The suit fits very well. This is not very practical, but it is not so easy for a cat to take off a suit. There is a small fabric "suit" underneath the cut that holds the costume to the pet. The front piece has a separate Velcro strap that goes around the pet's neck. I love this costume way more than my cat!
