They have a thin hollow post. The "design" is twisted so you can insert the pen from the back. The design is so incredibly tiny that I'm not sure a human could pull it off, maybe a real ninja kid with phenomenal agility, but adults, yeah I don't think so. If you're looking for small hole cutters this is it, otherwise I'd suggest stopping by.
