The new one has been working great for a couple of weeks now

Plugged in (120V AC outlet), smoked straight away and never worked again. Yes, I checked that the DC switch isn't on before plugging it into a 120V outlet. I never bothered to get a replacement because I just went to the store and bought another. The new one has been working great for a few weeks now.
