While I can't say it's a 'must have' like some of the other KIWI Design Valve Index products, it is a very simple but effective solution to keeping your Valve Index more hygienic and wear and tear to reduce. The leather only covers the fabric of the t-shirt that comes with the index, which is certainly comfortable but unfortunately absorbs sweat very easily, making it smelly and wearing out pretty quickly. Leather is a great option because when it sweats you can just wipe it off without worrying that it will soak up the sweat and turn into a kind of "sponge". yes, it's as rude as it sounds. This product is also great when sharing a headset with multiple people, especially when your HMD could end up on some fatter/very sweaty heads. This allows for easy cleaning between users and keeps the main material of the headset in perfect shape. If you actively play VR with Valve Index I would say this is a great addition for you!
