The large size is for a longer torso.
I have a short torso and ordered a larger or wider version. The belt didn't fit so well because it was too wrinkled. The material is soft, almost like fabric. The belt is flexible but too flexible. It's easy to use; It wraps around the waist just twice to tie in the front and is just long enough to accommodate a larger waist. My waist is 31 inches and I can put it on my back. Four stars because the material needs to be stronger.
