Under these test conditions, a blown cylinder head gasket allows high pressure air to enter one or both of the coolant or oil passages. This can cause the coolant expansion tanks to overflow. Compressed air can seep (and whistle?) through the oil filler cap and/or air/oil separator. This tester can easily detect a blown head gasket that a compression test would completely miss.
