Use your fingers. If this is not possible, try a similar product.

DISCLAIMER: Not a doctor, just a review based on my own experience of using this product three times a week and my own scalp/hair situation. Of course, I can't say anything about the long-term effects. I used it in the shower with shampoo. FEELINGS: Quite good, to be honest. I don't know what people were raving about. An uncomfortable and tingling feeling. I think it's too hard even with minimal pressure. You're better off having a wide-toothed comb or wet brush to use in the shower if you like the feel and use your fingers to massage. I recently bought a "wet brush detangling shower" advertised with "super soft Intelliflex bristles" which feels really good. DANDRUFF: I have chronic flaky scalp. Personally, after 3 uses it didn't help my dandruff, it actually got worse. HAIR TEXTURE: I think my hair is softer but also messier after using it. My hair is mostly 2b, high porosity. ✓ HAND PROBLEMS: I RECOMMEND this type of product (Silicone Scalp Massagers) to anyone experiencing pain in their arms, wrists or hands to reduce the amount of energy required to cleanse the scalp. (This is a bit like a fingers-only workout). This product is definitely easier on your fingers if you have hand problems. But I would have gone for a smaller scalp scrubber with a more rounded shape, this shape was too big for me to hold comfortably. Before purchasing, check the dimensions of the product to see if it fits your hand. And maybe look for one with big, rounded ends that are well spaced apart to better mimic your fingers and make your hair less tangled. I haven't tried any others so I don't know if I'm right that this style would be better. It's worth investigating if you're having the above issues. WATER TRAPPED: Water has trapped after repeated use, which is unhygienic. I think to remove the water you would actually have to break the product, which is undesirable. GENERAL: too rough for my scalp. Since my scalp was my main concern when I bought this product and it doesn't seem to be helping, I wouldn't consider it a success. I get the best results with my fingers. And many hairs got out and tangled. If you want to try this type of product because you have hand/finger issues I would look for something softer with rounded tips and make sure the size is right for your hand. If everything is fine with your hands, then it is better if you do not know how to wash your hair properly. What works for me is gently rubbing the shampoo into my scalp conditioner with flat fingers and then conditioning. Much luck!
