That's a long story. The day after I ordered this, ideally for my husband who has chronic back problems, I injured my back. pulled muscle? pinched spinal nerve? ejected the disk? broken rib? I don't know, except that it HURT and if I move wrong the muscles contract so much they literally pull my body into unnatural positions. So when it came, it landed on my body, not his. Firstly it seems well made, lots of details including a support bar on the back of the strap with a small pocket so you can use it with a cool backpack or add a small heat pad for local or prolonged heating. Part of the inflation is done manually. You get the pump and the belt mount is interesting and requires a little attention to get it right first. Because it's manual, it won't be affected by battery life, and if you're comfortable you can leave it on all day. The heating runs around the entire waistband and is not located on the back. The controls are simple and, like connecting the pump, require a bit of attention until you get used to which buttons are which. The warmth aspect is why I chose this belt over others. and as i stand here typing this, connect the heater block battery charging strap. You will get at least 2 uses of the heater at maximum power for the maximum time (30 minutes). i am overweight In fact, according to the actuarial tables, I'm obese. and the belt fits my waist without using an extender. I think my waist is about 47 inches. It's hardly noticeable, but it fits. An extender is just a piece of fabric with velcro on both sides so you attach one side of the belt to the lining and the other side/end to the other end of the belt. Should give you another 4, maybe 6 inches. I wear this over a fitted t-shirt to protect the strap from skin oils and my skin from irritation. At maximum temperature, it is NOT uncomfortable. As for inflating the belt to stretch the lower back - not so much. At least not for me. Maybe if it was tighter, but then it would be too uncomfortable for me to wear it for a long time. My back is injured higher than my lower back, just below my shoulder blades. So the belt is as snug as possible just under my chest and then I inflate it to give even pressure and support. And while my comfort level is still pretty uncomfortable, I don't have any severe cramping that I've experienced before. Picked everything up this morning, set it to max for 30 minutes and survived my dog walk. cool, damp morning in relative comfort. We were more than 30 minutes and once the heat was gone the discomfort was high enough to make me wince but again no major cramps. Since I didn't spend more than $100 on a pressure-only device, consider this a win. It can take several days and progress slowly, but I don't need any medication, not even aspirin, to go through this healing process. and again I bought it for the thermal aspect not the spine adjustment and it works perfectly! UPDATE: The small USB connector on the device came loose and fell into the box. No more charging! After contacting the seller I switched to one of those microwave heated pads. VERY FAST response and a new block sent immediately. EXTREMELY pleased with the customer service from this seller.
