So far so good. Unlike a few other "fanless" h4 leds I've tried, it hasn't burned out in a week. The beam pattern is good and mirrors the regular h4 very closely, with sharp definition in the low beam, and the high beam has good spread without all shining into the trees like some of the other lights I've tried (you could either aim at the low beam right or up, but not both). The fan runs quietly and is not audible when the engine is running. The "bulb" is slightly thicker than normal so it may not fit through the hole depending on the vehicle. At this point you can either go for a bulb with a fan/radiator on the back (not an option on many bikes, they don't fit most traditional bucket headlight housings) or slightly trim the Dremel housing. Luckily my Guzzi v9 had a removable hole expander insert which appeared to be some sort of passive heat barrier for very hot incandescent bulbs. With that removed this chunky h4 is great and while it still gets hot it's nowhere near as hot as a regular h4 so won't melt the plastic reflector housing or anything. But there your results may differ.
