Must use both picks and brushes. I didn't find the brushes small enough for some areas, so I use them for those areas. They massage the gums and feel great. I can even reuse them (by soaking them in a cleaning solution along with fixatives), but the massage heads are easily removable, so they can't be reused many times. I wish they weren't plastic and more like interdental rubber brushes. Can you see the little brush on the left side of my photo? That's all you throw away when using interdental brushes, even though the gum brush handle is plastic. This way you really generate very little waste. I look forward to chewing gum making a tiny head like this pick that fits onto the handle so we can use a multi headed handle.
