This belt has a medium sized stainless steel hook and measures 47.5 inches from end to end. The tag on the belt says it's a size 40. It has five holes, and the last hole from the top is the one I use for my 34-inch waist size; it fits perfectly. What's left is about six inches, which is also perfect. I folded the leather to check for creases and saw a mark on the inside but not the outside. It is of good quality.
