I immediately noticed a huge difference in terms of sleep and pain that I had previously experienced. Also no longer had my very stuffy nose every morning. All of these things were such a great relief from the problems I had had for years. My negative comment is that I wish it was a little firmer because it is very soft. I chose 4" because of my weight and wanted to make sure it made a difference. The lid of the topper can also be easily removed. I've had the mattress cover for a month now, the smell is still there. When I first opened it, it smelled like plastic or cigarettes. It's faded now, but it's still there.
