True to size on label for toddlers but the neck opening is really wide and big. Underneath we put on a t-shirt to accommodate the large opening for the neck. The kid didn't seem to mind. Nice thick cotton, but the striped sleeves don't have as much stretch as the yellow cotton part of the shirt. We've washed it twice in the cold since then and it hasn't shrunk or warped. The truck decal is really thick and originally had an odor when it was new. We only kept it because the little one saw it and immediately wanted to wear it.
