Medium size perfect for my 32lb / 18" / 20" dogs. It seems to help get rid of the mites, but you're still prone to them on your head and ears. I would give it 5 stars except the stitching comes undone almost immediately and my two dogs run and fall through the bushes and the flimsy material rips. The upside, however, is that it doesn't get too hot because it's thin. like that . . . at $30 it's not too bad even if you have to buy one every year - cheaper than a Seresto or Frontline collar and a lot less toxic. Both of my dogs have skin sensitivities so this is important.
