Charming shirt! The tie and the top really add something. I bought 5T for my daughter who is 4 years old. She weighs about 33 pounds, tall and thin. She's usually in between sizes, but because she's so tall I prefer going higher so her tummy isn't visible. The shirt is true to size. The 5T was a little big, but not so big that it couldn't be carried, which was expected when ordering. She could probably wear 4T as they are definitely true to size.
