I was so excited but why is there no warning!

I was so excited that this would happen because I live on my bike and I thought it would be great if my dog was with me. Spring. I attached everything to the bike and put my pup in it and tied him to the harness and fastened him then started to ride slowly and suddenly my pup stretched his arms out, stood up, fell out of it and HANGED for a few seconds! I had to hurry and pick it up! Why isn't there a warning or maybe something safer! I figured the straps would keep him safe and sound! I am feeling so bad!
