I use this with an Otter box. I wrapped it around one of the holes in the case. A part of the lanyard is thinner than the main strap, the retractable string will not block the hole. The tension of the retractor is pretty weak, so I don't know how well it protects in a fall, but that's why I keep my phone in a pocket. I can easily take my phone out of my right back pocket and put it in the dashboard mount. However, with such a length, it pulls the phone. I use it because my summer pants don't have very deep back pockets.
