I plan to put tucks with sewing thread into the round foam at the neck to pull it flatter. I had damage from the seat belt slamming my body and head against the back of the driver's seat during a bad auto accident last July. Sleeping against even my pillow hurts. I like one of this manufacturer's other neck support for during the day and purchased this softer one for sleeping. However, that top roll pushes strongly against the nerves at the back of my neck. I think the reduced pressure at the top will allow me to use it comfortably.
